Student loan forgiveness program.

A “severe permanent disability” is any impairment, including a physical, mental, intellectual, cognitive, learning, communication, or sensory impairment - or a functional limitation - that prevents you from participating in substantially gainful employment, and will remain with you for life.

  1. Complete the Severe Permanent Disability Benefit Application.
  2. Have a licensed physician or nurse practitioner complete the Medical Report—SPDB.
  3. Mail both to the Canada Student Loans Program,
